I need help completing the Land Registry form ST5 which I submitted with all other forms required to cancel a deed of trust. It has been returned to me seeking an answer to paragraph 4 and seeking further clarification on para 3 The deed was created because I paid the whole purchase price but wanted my partner to be on the deeds. The deed of trust ensures that if we split up (which we have) the property should be sold, and the profits split equally between us once the costs of buying and selling, including the £200,000 purchase price have been returned to me. He has now left, the property has not risen in value, and he wants to be off the deeds leaving me as sole owner, without selling the house (which I am intending to renovate and continue to live in.) Whilst both of us agree to this, the land Registry wants to know what has happened to the beneficial interest protected by the "restriction" and for panel 4 it is asking "how" the interest has passed. I confirm that there are no loans, debts or mortgage associated with the property, and no-one other than myself and my former partner have any claim to the property.

From your comments I assume you have tried to complete a Transfer of Equity (having previously owner the property as Tenants in common and have submitted the ST5 along with the TR1 and RX3/RX4 for removal or cancellation of the restriction.
It is very hard to advise what needs to be added without having a better understanding of the matter but it is very important that you get this done correctly to ensure you do not encounter any issues when you eventually come to sell the property on.
